You can grab the valentine baggie topper here.
It is formatted for a standard sized sandwich bag. Once you download the template, you can clip your own photo in and personalize your topper. Print, fold and trim. Then add your treat, staple topper to the bag and you're ready to deliver some fun valentines! Easy peasy! (If you are new to clipping masks you can read my tutorials here or here.)

I also have some very simple 3x4 cards, perfect for your project life or blog pages. You can dress them up with photos, ribbon or embellishments. Or leave them as they are. Your choice.
